    Description

    RARE Prohibition Era Physician's Record of Prescriptions, "Treasury Department, United States Internal Revenue, National Prohibition Act Form 1402," with Washington 1920 on bottom of the front cover. The book begins with a page of instructions, followed by tabbed pages (A to Z) with over 80 handwritten entries (dated 1926 to 1928) for prescriptions for whisky (whiskey), sherry wine, and spiritus frumenti.
